Balancing Act
A comprehensive fall-prevention and intervention program focusing on both risk reduction and appropriate interventions after a fall has occurred.
Restorative Nursing
TMC’s Restorative Nursing Programs help decrease the risk of re-hospitalization, improve quality measure outcomes, and capture all appropriate reimbursement opportunities.
Respiratory Therapy
This program is designed to assist communities in improving care to their residents managing respiratory issues, reducing rehospitalization due to pneumonia.
PRIDE
The Programs Reducing Incontinence Disorders and Embarrassment Program (PRIDE) is designed to work in collaboration with individuals to aggressively address factors which contribute to incontinence.

Mission Cognition
A Cognitive Rehab Therapy (CRT) Program with the primary goal of functional application of cognitive skills to improve a person’s ability to reintegrate into the community at the highest level achievable.
Neuro Connections
Encompasses treatment of any disease or trauma that results in dysfunction of either the central or peripheral nervous system, including best practices for dosage and therapeutic interventions.
InVENTive
Many aspects of life can be impacted by the presence of an artificial airway, but with strategic and focused rehabilitation, they do not have to continue to negatively affect quality of life.
Dysphagia
This program includes intervention, compensatory education, and best practice recommendations for managing the rehabilitation needs of patients with difficulty swallowing.
Comfort First
The Comfort First Program is focused on identifying and correcting underlying positional abnormalities and then addressing the resulting risks for decline related to acute illness or occurring as a result of an existing disease process.

TMC’s Positioning, Orthotics, Stretching, Exercise Program (POSE) is focused on identifying and correcting abnormalities causing contractures or risk for contractures in the long-term population.

Pain Busters is a comprehensive guide to the treatment of specific types of pain commonly encountered in the Skilled Nursing setting, as well as pain treatment in general.

From the moment an agreement is signed, our experienced transition team works closely with your leadership, clinical, and billing teams to manage every detail of the changeover—so care delivery is never disrupted.
We coordinate staffing continuity, clinical program setup, documentation workflows, compliance requirements, and communication plans to ensure your therapy services are fully operational from day one.
